Medicaid Service Coordination Program:
Our complex care system can be challenging and it can be difficult to connect with necessary support services. Our Medicaid Service Coordination Program works 1:1 with adults and children with developmental disabilities to create the best blueprint for achieving healthy, independent and active lives. Your Medicaid Service Coordinator will assist in creating an Individualized Service Plan (ISP) that states your goals and objectives and help pull in the resources to achieve them. Service Coordination can begin prior to qualifying for disability services.
