Assisted Housing

Shelter Plus care

Designed to maintain stable housing for low-income individuals with substance abuse and/or mental health diagnoses. Serves individuals who are homeless and have documented status of homeless per the U.S. Department of Housing and Urban Development (HUD). Shelter Plus Care provides case management service including an Individualized Service Plan (ISP) and monthly home visits. Participants must be actively engaged in treatment programs and will be linked to further community supports, as well as Section 8 housing. Assists the recovery process by ensuring housing is maintained through monthly rental subsidies and security deposits as needed.

Criteria: must be able to document homelessness, treatment and income. Case managers work with consumers to develop Individualized Service Plans (ISP) with goal of achieving self-sufficiency within 2 years. Case managers have weekly contact with consumers and biweekly home visits, work on linking to community supports. ISP usually addresses personal, vocational and housing needs.

OASAS Medicaid Re-Design Team Permanent Supportive Housing

The MRT Permanent Supportive Housing Program is a partnership between OASAS and Central New York Services, Inc. It was designed to enable individuals with chronic substance abuse issues to obtain and remain in safe and affordable permanent housing. Program components include rental subsidies and other occupancy costs for apartments, program supervision, housing counseling services, employment counseling services and, clinical supervision for the direct care staff. Criteria: must have a substance abuse diagnosis; must be actively on Medicaid; must have a history of 2 inpatient hospitalizations/rehab stays, or 5 emergency room/detox visits within the past 12 months, must be enrolled in or eligible for and willing to enroll in a Health Home; must be a single adult living alone; must have a history of or be at risk for homelessness.
