The Supported Housing Program provides independent apartment living for dually diagnosed adults, and furnishes varying levels of support determined by the desire of the individual and the needs defined by the referral source.

The staff provides housing set up and support to maintain permanent residence through community contacts, communication with landlords, financial sources and treatment teams.

Participants are not subject to outside program attendance or length of stay requirements. This program's goal is to offer individually based housing options to those presently under-served within the community and to maintain that housing through support and advocacy.

Currently there are three distinct but related programs operating under the heading Supported Housing. They are the "traditional" Supported Housing Program, the Housing First Program, and the Intensive Supported Housing Program, designed for transitioning persons previously in long term care at state psychiatric facilities.

Individuals interested in this program must obtain a SPOA (Single Point of Access), which in Onondaga County is administered by Onondaga Case Management, (click on the link or call Jan Moag, SPOA Coordinator at 315-472-7363 for more information).
